Noticeboard

Our Facebook page has lots of useful information and updates.  Make sure you 'like' us to keep up to date

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • non_nhs_services1Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Passport signing
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Insurance and Legal Reports

Most insurance reports can be completed from your medical records. Occasionally some companies ask for us to carry out a detailed medical examination. They will let you know if this is the case.Always make sure you have given us written consent to complete a report about your health. We treat your medical information with strict confidentiality and only pass it to others with your permission.

Forms for Benefits and Social Security

If you apply for a benefit (such as Disability Living Allowance, Attendance Allowance, a disabled driving badge) please do not ask us to fill in your initial application form. There is a misleading part on many of these forms where it asks for ‘someone who knows you best’ to describe how your disability affects you. This is for a friend or relative to complete. We are sent our own medical form to complete when your application has been submitted.

If you are having problems filling in an application form (they can be confusing) please contact Welfare Rights for help. They are specially trained to make sense of these forms. Phone 342 3068.
