New Patient Registration

We are currently open to accept new patients to our list who live in the area defined by the map below. There are other local practices covering the other areas of Stalybridge.  Everyone is welcome in general practice. You do not need to proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.

If you decide you would like to join our list we would ask that you complete a Registration Form and a New Patient Medical Form. This asks for information such as your medical history, your height, weight, smoking habits etc. These forms can be collected from Reception at the practice.  During the COVID pandemic, please phone first to discuss how you can regsiter with us safely.

If you are on regular medication you will be asked to make an appointment with one of our GPs before your next prescription is due. This is so we can better understand your need for that medication. When you join us we request your medical notes from your previous GP and this may take several weeks.


All new patients accepted to our list live in the following area. If you are already one of our patients and move out of this area you may be asked to register with a new GP. We feel it is important our patients live close to the practice in case you are seriously unwell and we need to get to you as quickly as possible.
