Annual MOT Check
If you have one of the following medical conditions we like you to attend for an annual Birthday MOT during your birthday month.
If someone is very unwell, or approaching the end of their life, we will call you to discuss our preferences.
We will carry out many tests and checks
Blood tests Kidneys, liver, thyroid, cholesterol, blood sugar (some people may also need a gout test (uric acid), PSA, (prostate) or blood count)
Urine test To check how healthy your kidneys are (please bring an early morning sample)
Blood pressure To check you are not at risk of strokes or heart attacks
Height and weight To check your general health. We will also ask you about smoking and alcohol and the amount of exercise you take.
Vaccinations If you are due any immunisations, such as a flu jab, boosters or shingles vaccination we will offer these.
Foot and eye checks If you have diabetes we will check you are having regular foot and eye checks.
Before your birthday month we will invite you to an appointment (for example in April we will invite every eligible who has a birthday in May). Initially, we will give you an appointment time and date, but if this is not convenient, please call us to rearrange. The first appointment will be with our Health care Assistants to get the tests done.
The Health Care Assistant will discuss with you the best way for you to receive your results. For some conditions we will ask that you attend a second appointment with the Practice Nurse who will produce a written plan on how you would like to improve your overall health and manage your long term condition.
You may prefer to receive your results by letter or email (for example if you are feeling fit and well and receiving optimum treatment for your condition). We will ensure that you are fully informed when your next checks are due and what they will be.
