GP practices in Stalybridge Primary Care Network have already identified those patients who meet the criteria and they are now waiting for the final service specification from Greater Manchester ICB (the organisation that is responsible for planning the health services for our local populations) Once this is finalised, if you are eligible, we will get in touch as soon as possible.

❗️The criteria to qualify is as follows:

If BMI is above 40 then you will also need to have been diagnosed with four of the following five criteria:

  1. Diagnosed hypertension (High blood pressure)
  2. Diagnosed sleep apnoea
  3. Diagnosed type 2 diabetes
  4. Diagnosed cardiovascular disease (Heart disease)
  5. Diagnosed hypercholesterolemia (Abnormal blood fats)
