Postnatal Information for New Parents

CONGRATULATIONS on the birth of your baby.

Please make a note of the date of your Postnatal and 1st baby immunisation appointment which will be sent to you in the post along with a purple form. This is for you to complete with the baby’s details and then return to us so that we can register the baby with the practice.  It is essential that you try and keep this first appointment to start the course of immunisations for your new-born baby. If there is anything you wish to discuss prior to that appointment please do not hesitate to contact the surgery via our online consultation form.

IT IS IMPORTANT THAT THE PURPLE FORM IS RETURNED TO THE PRACTICE AS SOON AS POSSIBLE SO THAT BABY’S DETAILS CAN BE RECORDED ON OUR COMPUTER SYSTEM. YOU DO NOT HAVE TO WAIT UNTIL YOU HAVE REGISTERED THE BABY WITH THE REGISTRAR.
